Medical Insurance Program - AllState Benefits:

Allstate Benefits' Health Savings Qualified medical plan effective December 1, 2024, is designed to provide comprehensive coverage while offering the advantages of a Health Savings Account (HSA). As a Health Savings Qualified program, it combines a high-deductible health insurance plan with the option for you to set up a tax-advantaged savings account.

Employee Only Medical: You pay $79.03 per month.

Employee & Spouse Medical: You pay $201.52 per month.

Employee & Child Medical: You pay $154.10 per month.

Family Medical: You pay $260.79 per month.

Vision Insurance Program - Guardian Benefits:

Vision insurance is a way to reduce your eye care expenses. In exchange for a monthly premium, this plan picks up some of your costs of vision care such as lenses, frames, and contact lenses.

Employer Contribution: Your employer covers 100% of the total cost of this vision insurance program.

Life Insurance , & Disability Insurance - Guardian
